CHIP
requires small co-pays as well as premiums
of up to $75.00 per family every three months.
The amount of the premium varies depending
on income. No family on CHIP will be required
to spend more than five percent of their family's
income on premiums, co-pays, and other covered
services over the course of a plan year. American Indians do not have to pay premiums or co-pays for services. |

The Department of Workforce Service (DWS) Business Office will send an invoice when your premiums are due. You will receive a new invoice every three (3) months, starting with the first month that your child enrolled in CHIP
|

If your Premiums are not paid and your CHIP coverage is going to be canceled, you will be notified. Families with closed accounts for non-payment may come back onto CHIP if they pay within 30 days. Once benefits are canceled you will have to pay the past due balance before re-applying. |

Why
pay your premium on time? |
| |
It is very important that you pay your premium on time so there is not a delay in your child's healthcare.
There’s no question that your CHIP coverage
is current when you pay on time.
If you do not pay it by the due date, a $15 late fee will be changed to your account. You will then have to pay the premium and the late fee to keep your child on CHIP. If you do not pay your premium, your case will be closed. |

cHIE |
| |
Medical assistance (Medicaid, CHIP, UPP, PCN) recipients are automatically enrolled in the Utah Clinical Health Information Exchange (cHIE). The cHIE provides a safe place for participating healthcare providers to share and view patient medical information.
Once your consent status has been set to PARTICIPATE, it will remain in effect for five years or until you turn 18 if you are a minor. Recipients have the right to not participate in the cHIE or to change their consent status at any time.
