CHIP
requires small co-pays as well as premiums
of up to $60.00 per family every three months.
The amount of the premium varies depending
on income. No family on CHIP will be required
to spend more than five percent of their families
income on premiums, co-pays, and other covered
services over a the course of a plan year. American Indians do not have co-pays for services. |

CHIP
mails quarterly premium invoices the first
week of February, May, August and November.
Premiums are due about one month later. If your Premiums are not paid and your CHIP coverage is going to be canceled, you will be notified. Families with closed accounts for non-payment may come back onto CHIP if they pay within 30 days. Once benefits are canceled you will have to pay the past due balance before re-applying. |

not delay in your child’s healthcare.
There’s no question that your CHIP coverage
is current when you pay on time.
If you don’t pay your premiums on time,
you run the risk of being automatically dropped
from the program. |
