Satellite Clinic Schedule

Children with Special Health Care Needs (CSHCN) provides evaluation clinics for children with special health care needs in rural Utah. These clinics work in collaboration with the child's Medical Home (Primary Health Care Provider).

Evaluations are provided by multidisciplinary teams which travel to 8 sites across the state. Clinics feature specialty providers in the following areas:

Developmental Pediatrics
Nursing
Social Work
Psychology
Speech and Language
Audiology

Other pediatric evaluations may include: physical and occupational therapy, neurology and genetics.

Satellite CSHCN nurses and staff are available for the following sites:

Logan (Neurology evaluations only)
Moab
Ogden
Price
Richfield
St. George
Vernal

Providers, please download the CSHCN Referral Form. Once completed, give a copy to the patient and fax the form to the local clinic.
