Assessment of Glycemic Control:

Hemoglobin A1C and Self-Monitoring of Blood Glucose –
Indications, New Technology, and Effective Utilization

Target Audience:

This activity has been designed to meet the needs of physicians and other healthcare professionals who are involved in the diagnosis, management and treatment of persons with diabetes.

Educational Objectives:

At the end of this activity, participants should be better able to:

  • Assess glycemic control using patient A1C and blood glucose data
  • Apply findings from the International A1C-Derived Average Glucose study

Estimated Time of Completion: 1.25 hours
One hour to view the video-on-demand program or DVD and 15 minutes for evaluation and post-test.

Method of Participation:
To receive a statement of credit, you must:

  • View the entire one-hour program either through the Utah Telehealth Network “Video Archive of 2008 Presentations - September 17, 2008” session by clicking here or by requesting a DVD copy from the Utah Diabetes Program at (801) 538-6248.
  • Successfully complete the evaluation form and post-test (80% or higher).
  • You will have an unlimited number of attempts to successfully complete the activity.
  • Upon successful completion, a certificate of participation will be sent to the email provided on the evaluation form.

Your statement of credit will be recorded with the U of U CME office upon successful completion of the post-test and submission of the evaluation form.

Fee: This activity is provided at no cost to the physician unless an immediate certificate of credit is requested.

Accreditation: The University of Utah School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

AMA Credits: The University of Utah School of Medicine designates this educational activity for a maximum of 1 AMA PRA Category 1 Credit(s)™.  Physicians should only claim credit commensurate with the extent of their participation in the activity.
