Telehealth Archives and Continuing Education

You can receive continuing education credits for past Diabetes Telehealth Programs.at no cost just like our live programs. 1.5 hours for nurses and 1 hour for dietitians are available for several archived programs if you did not participate in the original live program.

To receive credits follow these 5 easy steps:

1. Gather the following THREE materials by downloading and printing from our website (see desired program link below):

• PPT handout
• evaluation form
• post-test

2. View the archived program using Windows Media Player following the link on our website to the Utah Telehealth Network or request a DVD from Nathan Peterson.

3. Complete post-test ( must pass with 80% ) and evaluation form

4. Return post-test/evaluation form by email, mail or fax to Nathan at the Utah DPCP

5. Receive a certificate of completion with CE credit by email
