♠ Retest fees: See below. For practical tests, retest fees apply only to sections being retested.

♣ An additional $35.00 fee to perform a federal background check will be charged for those who have lived out of the State of Utah in the last five years.

♥ An additional $15.00 fee will be charged for lapsed certifications.

♦ The Paramedic initial written test is administered by the National Registry of Emergency Medical Technicians. The test fee is determined by and paid to NREMT.

◊ If recertification practical testing is administered by a recognized testing agency other than the State test team, fees may vary according to that agency's testing prices and would be paid directly to that agency.
