EMS Instructor Seminar

Dates, Location, and Cost

Dates: August 7–8, 2009

Times: August 7: 8:00 am–4:30 pm
August 8: 8:00 am–12:30 pm

Location: The Yarrow, Park City, Utah
Cost: $150 ($175 after July 30, 2009)

Requirements

  1. Must be currently certified in Utah as an EMD, EMT, EMT-I, EMT-IA, or Paramedic (you must submit a front and back copy of your certification card with your application).
  2. Must be a currently certified CPR Instructor (you must submit a front and back copy of your CPR Instructor card with your application).
  3. Must be a currently certified EMS Instructor.
  4. Must recertify every two (2) years to remain current.
  5. Complete U-Train registration (see link below) and submit all required documents prior to the due date. Registrations and/or documents received after the deadline will be subject to a $25.00 late fee.
  6. Incomplete registrations will be denied.
  7. Documents must be mailed or hand delivered; no faxes please.

Application

Applications will be accepted through July 30, 2009, or until the course is full.

  1. Register on U-Train (Course ID 1017528).
  2. Submit supporting documentation (see instructions on U-Train course page).
