New EMS Instructor Course
Dates, Location, and Cost
Dates: November 2–4, 2009, 8:00 am–6:00 pm
Location: The Red Lion Hotel, 161 W 600 S, Salt Lake City
Cost: $150 ($175 after October 23, 2009)
Fire Instructor I or II: Attend Day 1 only
Fire Instructor I or II: $50 ($75 after October 23, 2009; you must submit a copy of your Fire Instructor certificate)
Requirements
- Must be currently certified in Utah as an EMD, EMT, EMT-I, EMT-IA, or a Paramedic for at least one year (you must submit a front and back copy of your certification card with your application).
- Must be a currently certified CPR Instructor (you must submit a front and back copy of your CPR Instructor card with your application).
- Submit three (3) letters of recommendation outlining and describing your EMS skills and teaching abilities.
- Submit documentation to verify you have fifteen (15) hours of instruction experience.
- Submit documentation to verify that you have provided thirty (30) hours of actual patient care within the past year prior to application (EMT, EMT-I, EMT-IA, or Paramedic only).
- Complete U-Train registration (see link below) and submit all required documents prior to the due date. Registrations and/or documents received after the deadline will be subject to a $25.00 late fee.
- Incomplete registrations will be denied.
- Documents must be mailed or hand delivered; no faxes please.
- Space is limited—only two persons per agency.
Application
Applications will be accepted through October 23, 2009, or until the course is full.
- Register on U-Train (Course ID 1019333).
- Register on U-Train—Fire Instructor I or II (Course ID 1019334).
- Submit supporting documentation (see instructions on U-Train course page).
