How to Submit a Story

  • Thank you for your interest in the Utah Diabetes Prevention and Control Program's Faces of Diabetes campaign! We believe that your story is valuable and would love to hear from you.

    Right now, we are collecting stories to both feature on our Faces of Diabetes YouTube Channel, and in our upcoming User Submission category. And if filming your story seems daunting, we're even accepting text submissions!

    So if you want to submit YOUR diabetes story to us, here's how!

    (A few tips before you begin: remember to keep it brief and keep it real.)

  • To Submit a Text Story


    OR

    1) Type your story in a word processing program (Microsoft Word, or Notepad are preferred)
    2) Save your story
    3) Email Grant Sunada at the Utah Diabetes Prevention and Control Program at gsunada@utah.gov and include your file as an attachment
    4) Read the Submission Agreement located at THIS LINK, then copy and paste it into the email, if you agree to it (this gives us permission to use your story)
    5) In the body of your email, please include your NAME, AGE, PHONE NUMBER and ADDRESS (if under 18, please include parent's contact info)
  • To Submit a Video Story

    To successfully submit your video, you must post a video "response" to the Gen's Story video found at THIS LINK. This simply means that your story video will be officially linked to our channel. You don't have to respond, in the traditional sense, to Gen's Video - you can tell your own story in your own way!

    You must also include your first name and last initial in the name of your video so that we know who's video is officially registered! Then E-mail Grant Sunada at the Utah Diabetes Prevention and Control Program:

    1) Send the E-mail to gsunada@utah.gov and include a link to your video
    2) Read the Submission Agreement located at THIS LINK, then copy and paste it into the email, if you agree to it (this gives us permission to use the video
    3) In the body of your email, please include your NAME, AGE, PHONE NUMBER and ADDRESS (if under 18, please include parent's contact info)

    In your video, tell us your story. You can be serious, funny, creative or classic - just keep it real and share with us your experience with diabetes.

  • To Post a Video Response:



    1) Register or Log-In to YouTube - CLICK HERE
    You must be signed into an account on YouTube to upload your video, and post it as a response. It's free and easy.


    2) Upload your video

    Once you are logged into YouTube, on any YouTube page you will see a yellow button marked Upload on the upper right corner of the screen. Click this and follow the instructions to upload your story into your YouTube account.

    3) Submit Your Video - CLICK HERE
    Once at this page, click the grey "Choose a Video" tab at the top. Then select the video you just uploaded from the list that appaers in the center of the screen and click the "Use this video" button.You should then see a message that confirms that you have submitted your video as a video reply.
