Please read all of the following information before proceeding.

1. It is important for you to complete all sections of this form so your complaint can be fully evaluated. If a particular section does not apply to your situation, simply write "N/A" (not applicable) in the space.

2. Your complaint will be treated as confidential. Under the law, complaints concerning the violation of Utah's licensing laws are not public information.

3. If you wish to contact the Bureau of Child Care Licensing by phone, please call (801) 538-9084.  Bureau hours are Monday through Thursday (except legal holidays) from 7:00 a.m. to 6:00 p.m. Mountain Standard Time.

4. Please be advised that the Bureau of Child Care Licensing will not act as your attorney for purposes of settling, recovering monetary loss, or performing any other dispute resolution between you and the facility you feel may have violated Utah's Child Care Licensing rules.

5. The Bureau of Child Care Licensing cannot perform investigations for the benefit of your personal litigation. However, your complaint is very important to the Bureau of Child Care Licensing because it informs us of possible violations of the licensing rules. Public complaints are valuable in helping to correct health and safety violations in child care facilities that are unlicensed or otherwise in violation of Utah law. All complaints are given very careful attention.

6. Once your complaint is reviewed, you may be contacted by someone from the Bureau of Child Care Licensing if more information is needed.

If a person knowingly gives false information to the Bureau for the purposes of changing the status of a child care license or certificate, the person giving the false information may be guilty of a Class B Misdemeanor. It is not necessary for you to be able to “prove” that the information being shared is true, only that you do not knowingly give false information to the Bureau.
