Evidence-based Pharmacotherapy Review Project

Welcome to the Evidence-based Pharmacotherapy Review Project. The goal of this project is to improve the health of Utah Medicaid beneficiaries by providing un-biased, non-commercial information and support to local providers. This information and support may include:

  • Identification of potential at-risk patients using computerized surveillance methods to screen for possible medication-related safety issues.
  • Clinical drug utilization reviews of identified high-risk patients.
  • Feedback on patients’ behavior, as well as evidence-based recommendations.
  • Medication Therapy Management Services provided to your patients at no cost.
  • Clinician lead Process Engineering workshops with CME credit conducted at your clinic free of charge.

Process Engineering
Research suggests that physician prescribing practices benefit from educational outreach and that effecting change in practice is helped by guidance in process improvement. The process engineering part of the Evidence-based Pharmacotherapy Review program aims to help physicians assess their improvement needs and to provide resources to assist in caring for Medicaid patients while obtaining AMA PRA CME credit.

Medication Therapy Management Services (MTMS) Medication Therapy Management Services may be offered at no cost to your patients. The aims of the MTMS program are to reduce adverse drug events by educating patients on appropriate drug use and the importance of compliance; and facilitate collaboration between patients, physicians, and pharmacists. Clinical pharmacists in the area have been certified to administer MTMS through The Pharmaceutical Care Certificate Program administered by Medication Management Systems, Inc.

Telephone
A telephone assessment with a brief education component may be offered for selected patients. The primary goal of this assessment is to determine if the patient needs a more intensive intervention. Patients needing more intensive interventions will either be referred to their primary care provider, an MTMS pharmacist, or both. All patients will be provided the appropriate educational materials by both phone and mail based on their assessment results.
