• Need to change Health Care Providers?
  If you live in the Wasatch front (Salt Lake, Utah, Davis, Weber, and Morgan Counties), call the Health Program Representative in your local Medicaid Eligibility Office.
  If you live in Tooele or Summit County, call Medicaid Information, toll-free 1-800-662-9651.) 
  If you live in Box Elder or Washington County, call your local County Health Department.
• Locate a Health Program Representative in your area.
• Mental Health Services: For information about covered mental health services, call the mental health center listed on your Medicaid Card. If the center identified on your card is not correct, call your Medicaid worker at your local Medicaid Eligibility Office. Also see Member Handbooks for Wasatch Mental Health, Northeastern Counseling Center, Four Corners Community Behavioral Health, Davis Behavioral Health, Davis Behavioral Health - Spanish version, and Central Utah Counseling Center.
• Insurance information: The name of your Third Party Liability (TPL) insurance is on your Medicaid card. To change this information, call the Office of Recovery Services, Team 76:  Salt Lake City area 536-8798 or call toll free in Utah:  1-800-821-2237.
• Compare CHIP, Primary Care Plan, Limited Medicaid Plans: chart which helps to explain co-pays and co-insurance fo each program.
• Concerns or Complaints:   Call 538-6417.  To call toll-free, call Medicaid Information and ask for Randa Pickle, or send email to randapickle@utah.gov 
• Estate Recovery - For clients age 55 and older, Medicaid may put a lien on a client's estate in order to collect back the amount Medicaid pays for the client's medical bills.
• Being billed for medical services? Please refer to the Medicaid Member Guide, or to the Utah Medicaid Provider Manual, SECTION 1, Chapter 6 - 8, Billing Patients, and 6 - 9, Exceptions to Prohibition on Billing Patients.  SECTION 1 is very long.  Be patient if you want to look at it. • Can you be charged for services not covered by Medicaid? Please refer to the Medicaid Member Guide, or to the Utah Medicaid Provider Manual, SECTION 1, Chapter 6 - 8, Billing Patients, and 6 - 9, Exceptions to Prohibition on Billing Patients.
