information: The name of your Third Party Liability
(TPL) insurance is on your Medicaid card. To change
this information, call the Office of Recovery Services,
Team 76: Salt Lake City area 801-536-8798 or call

- Estate
Recovery - For clients age 55 and older, Medicaid
may put a lien on a client's estate in order to collect
back the amount Medicaid pays for the client's medical
bills.

- Will Medicaid enrollees have their medical information stored and accessed through the cHIE? Medical assistance (Medicaid, CHIP, UPP, PCN) recipients are automatically enrolled in the Utah Clinical Health Information Exchange (cHIE). The cHIE provides a safe place for participating healthcare providers to share and view patient medical information.
Once your consent status has been set to PARTICIPATE, it will remain in effect for five years or as long as you are enrolled in PCN or Medicaid. You have the right to not participate in the cHIE or to change your consent status at any time.
