How to Use the List of Utah Medicaid Provider Manuals

quick start: Click on name of manual to open current manual. Click on plus sign (+) to see earlier versions of the manual, attachments and other chapters.

The manual won't open! . . . Most computers come with software for Adobe Acrobat Reader already installed. If your computer does not have this free software, it must be installed. It is easy to do yourself. Use the link at bottom of the page to go to the Adobe web site. It has instructions on how to download and install Acrobat Reader.

I get a "404" error message. Send an email, saying exactly what manual or attachment failed to open, to medicaidops@utah.gov. This error should be fixed within two business days.

1. How is the Utah Medicaid Provider Manual organized? . . . Each manual has at least three parts: Section 1, General Information; Section 2, Provider specific information; and General Attachments. Manuals may have more than one type of provider specific information, and they may have a section for Non-Traditional Medicaid Program. For more information on any of these, please see Utah Medicaid Provider Manual, Table of Contents .

2. How do I find specific types of policy, like physician, dental, home health and so on? . . . The list of manuals is generally in alphabetical order by provider type. Scroll down until you find the manual you want in the first column.

3. What are the month and year dates? . . . . These are the dates a new version of a manual or code list was issued. Generally, each page in the document is dated, and a vertical line in the left margin marks what changed on that page at that time.

4. What are ATTACHMENTS? . . . . Many manuals have code lists and forms which are specific to a type of service. There may be archives for earlier versions of these.

5. What is the the Non-Traditional Medicaid Program (NTM) ? . . . The Non-Traditional Medicaid Plan (NTM) is similar to Medicaid, but it has more limits and fewer covered services. The limits are described in the NTM Section of the Medicaid Manual. For more information about the program, refer to SECTION 1: GENERAL INFORMATION , Chapter 13 - 8 Non-Traditional Medicaid Plan.

6. Who do I contact if I need more information or help? . . . Call Medicaid Information (toll-free 1-800-662-9651). If you want a printed copy of a manual, you can also fax or mail a Publication Request Form. You can also send an email to Medicaid Operations at medicaidops@utah.gov.
