SECTION 1: GENERAL INFORMATION
SECTION 1 of the Utah Medicaid Provider Manual contains policy, procedures, codes and billing instructions which apply to all health care providers enrolled in the Utah Medicaid Program.

SECTION 2: PROVIDER SPECIFIC INFORMATION
This section of the Utah Medicaid Provider Manual has policy, procedures, codes and billing instructions specific to a health care service covered by the Utah Medicaid Program. You can download any of the manuals and code lists. View list.

NON-TRADITIONAL MEDICAID PLAN
A section covering the Non-Traditional Medicaid Plan is included with selected Medicaid Manuals. This section is included if the type of service is covered by the Non-Traditional Medicaid Plan.  The list of Medicaid Manuals by provider type has a column to show which manuals include the Non-Traditional Medicaid Plan
