MDS Case Mix Preview Report

Value of the Report
- The report will provide you detailed information relating to the case mix for your facility’s next quarter’s rate
calculation. You will be able to see what records were included in the calculation and other records that were outside the
period of interest (period of interest end date minus 275 days). As needed, you could then submit corrections to previously
submitted assessments. If received in time, the corrected assessment records would be included in the final case mix calculation.
- If you submit corrections to your MDS data, please submit them through the normal process – do not send them to the Reimbursement Unit.
- Note: If MDS corrections are submitted, they must be received in time to be included in the data extract for the final calculation.
- For example: The April 1 case mix is calculated using data that was extracted on, or about, the last business day of
February. As such, your facility should submit its changes well before the data extract date.
When to request the Case Mix preview report
- The report is not available until approximately two months prior to the rate effective date.
- See "Timing of Requests" table at the bottom of this page.
How to request the report
If your facility received a report in the past, the division must have received a letter stating that the previously
received report was destroyed. Please use the template provided below under "Destroying the report."
- Download the current Case Mix Preview Report (Word)
- Enter all the information requested on the form.
- It is easiest to move between the fields by using the "tab" key.
- Print the form on your facility’s letterhead.
- Mail the form to the division as addressed on the form.
Destroying the report
This "destruction notice" letter must be received within 60 days of the "report date" as noted on the
report. Failure to do so will preclude your facility from receiving future preview reports.

- Print the form on your facility’s letterhead.
- Mail the form to the division as addressed on the form.
Caution!!!
- The preview report contains confidential health information that is legally privileged.
- The preview report is intended only for the use of the individual(s) specified in the report header.
- Unauthorized re-disclosure or failure to maintain confidentiality could subject you to penalties described in federal and state law
(i.e., $10,000 per record - not report, but each line of the report).

| When should you destroy the report?***
| Within 60 days of the report date, it should be destroyed and you should have notified Medicaid,
in writing, that it is destroyed
