Apply in July! PCN will be open for all adults July 1 - 31, 2008. If you are an adult ages 19-64 and uninsured, you may qualify for PCN.
You may apply for PCN in three ways:
• Apply online
• Print and mail a PCN application. Applications must be received during Open Enrollment
•
• Stop by your local office
If you are uninsured, but you have access to health insurance through your employer, Utah's Premium Partnership for Health Insurance (UPP) may be able to help you pay for it.
